Step 3 | Print and sign the medical questionnaire (Bring these with you on the day of your DSD HERE
IMPORTANT: if you answer "yes" to ANY of the medical questions, you will need a doctor's note.
Step 4 | Show up 10 minutes early ready to have fun diving! You only need your swim suit and a towel. Many find that wearing a water t-shirt is more comfortable.
